\"بحران دولت رفاه\" به موضوعی مد روز برای پروژه های تحقیقاتی، کنفرانس های دانشگاهی و بحث های سیاسی در دهه های اخیر تبدیل شده است. با گذشت ربع قرن از ظهور به اصطلاح "انفجار هزینه های مراقبت های بهداشتی"، این باور عمومی وجود دارد که در بحث تامین مالی مراقبت های بهداشتی، بدترین زمان ها هنوز در پیش است. هیچ کس باور نمی کند که می توان بار فزاینده ای را بر دوش نسل های آینده کار گذاشت بدون اینکه واکنش نامطلوب آنها به خطر بیفتد. یکی از راه حل های اصلی پیشنهاد شده برای جلوگیری از بحران قریب الوقوع مراقبت های بهداشتی تحت عنوان "جیره بندی" قرار می گیرد و بنابراین - جای تعجب نیست - بحث جهانی در مورد اینکه آیا و چگونه به جیره بندی مراقبت های پزشکی پرداخته شده است اخیراً بسیار پر جنب و جوش شده است. جلد حاضر نتیجه کنفرانسی است که به موضوع جیرهبندی در پزشکی توسط آکادمی اروپائی zur Erforschung von Folgen wissenschaftlich-technischer Entwicklungen Bad Neuenahr-Ahrweiler GmbH سازماندهی شده است. یکی از اهداف این جلد کمک به شفاف سازی مفهوم سهمیه بندی و اجرای احتمالی آن و در نتیجه کمک به شرکت کنندگان در بحث برای جلوگیری از سردرگمی های غیر ضروری بیشتر است. نویسندگانی از رشته های پزشکی، فلسفه، اقتصاد و حقوق و همچنین دست اندرکاران خود مراقبت های بهداشتی و از حوزه تامین مالی مراقبت های بهداشتی در این جلد مشارکت داشته اند.
"The Crisis of the Welfare State" has become a fashionable topic for research projects, academic conferences and political debates in recent decades. A quarter of a century after the emergence of the so-called "health-care cost explosion", there is a widespread belief that in the issue of health care financing the worst times are still ahead of us. Nobody believes that it will be possible to place an ever increasing burden on future working generations without risking their adverse reaction. One of the main solutions proposed for averting the imminent health care crisis comes under the heading of "rationing" and thus - not surprisingly - the world-wide debate on whether and how to ration medical care has become very lively recently. The present volume is the result of a conference devoted to the topic of Rationing in Medicine that was organized by the Europäische Akademie zur Erforschung von Folgen wissenschaftlich-technischer Entwicklungen Bad Neuenahr-Ahrweiler GmbH. One of the purposes of this volume is to contribute to a clarification of the concept of rationing and its possible implementations and thereby to help the participants in the debate to avoid further unnecessary confusion. Authors from the disciplines of medicine, philosophy, economics and law as well as practitioners of health care itself and from the field of health-care financing have contributed to this volume.
